Commissioner for Human Rights

Instruction:

The following text is a template of an application to the Commissioner for Human Rights.

Pursuant to Article 10 of the Act of 15 July 1987 on the Commissioner for Human Rights  (Dz. U. [Journal of Laws] of 2001, No. 14 item 147), an application addressed to the Commissioner shall be exempt from charges, and no special form shall be required; however, the identity must be indicated of the applicant and the person whose liberty and right is involved in the case, and the subject of the case must be defined.

No power of attorney is required in order to submit the application to the Commissioner for Human Rights (information for lawyers and legal advisers).

The application should include the applicant’s identification (name, surname, street, house number, postal code and town/city). This allows to register the case, to find it in the system and reply by the Commissioner for Human Rights.

Pursuant to Article 24(1) of the Act of 29 August 1997 on the protection of personal data (Dz. U. [Journal of Laws] of 2002, No. 101, item 926 as amended), the Commissioner for Human Rights, with the registered office in Warsaw al. Solidarności 77, is the data controller.

To allow the Commissioner for Human Rights to examine a given case, the data are processed according to the Act of 15 July 1987 on the Commissioner for Human Rights (Dz. U. [Journal of Laws] of 2001, No. 14, item 147).

The data subject has the right to access his/her personal data and to correct them.
