Instruction for the electronic motion.
Choice of motion
There are two versions of the form available.
A form for the petitioner themselves - a person writing to the Commissioner for Civil Rights Protection on their own behalf.
A form for the petitioner writing on someone else's behalf - a person writing to the Commissioner for Civil Rights Protection on someone else's behalf.
Filling in of the motion
Type the data in order to fill in the fields in the form. Some fields are marked as "mandatory", e.g., name, surname, street etc. Such data is indispensable for the Commissioner for Civil Rights Protection in order to send an answer to the petitioner when their case has been examined.
An unlimited amount of text can be written (pasted) in the "Text of the motion" field.
If the mandatory fields have not been filled in, the following information appears
"Please fill out this field." It means that the motion has not been sent.
Attaching files
To add a file, click the "browse" button and select one or more files. Depending on the speed of the link, the size of the file being transferred, and the load on the server, the time it takes to attach attachments may vary and may take several minutes. After adding the file, another field will appear with a "browse" button in which we can add another attachment.
The size of a single file cannot exceed 20 MB. The number of attachments cannot be greater than 20 items. The size of the submitted application may not be greater than 120 MB (attachments with the text motion).
The following attachments are allowed: .7z, .avi, .css, .csv, .dgn, .doc, .docx, .dwf, .dwg, .dxf, .geotiff, .gif, .gml, .gz, .gzip, .html, .jp2, .jpeg, .jpg, .m4a, .mp3, .mp4, .mpeg, .mpeg4, .mpg, .msg, .odp, .ods, .odt, .ogg, .ogv , .ott, .pdf, .png, .ppt, .pptx, .rng, .rtf, .svg, .tar, .tif, .tiff, .tls, .txt, .wav, .xhtml, .xls,. xlsx, .xps, .xsd, .xsl, .xslt, .zip, .zipx.
Attachments added before sending the application can be removed by clicking the "X" next to the file.Sending the data entered
When you make sure that all data entered is correct and all files have been attached, click the "Submit the motion" button. The data will be sent and a confirmation of submitting the motion will be generated. The data appearing on the monitor can be printed by clicking on the "Print confirmation" button in order to save a copy of the motion sent to the Commissioner for Civil Rights Protection.
Important! If any files have been attached to the motion, only their names will be listed on the confirmation. The contents of the attachments will not be printed.
